Thank you for your interest in Calvary Christian School. Acceptance into CCS is granted to students whose parents sincerely desire a biblically based education for their children. The object of Calvary Christian School is to obey the scriptural command of Deuteronomy 6:5&7.

"And thou shalt love the Lord thy God with all thine heart, and with all thy soul, and with all thy might. And these words, which I command thee this day, shall be in thine heart. And thou shall teach them diligently unto thy children."

Calvary Christian School is a ministry dedicated to the glory of God. It exists to help students grow spiritually, emotionally, physically and academically. Students attending CCS are expected to govern their lives at all times to be pleasing to God. Students admitted to CCS will be expected to represent the school well and to be an example of Christian conduct and attitude.

Each student applying for admission will be expected to adhere to all principles outlined in the Parent/Student Handbook, and not to participate in activities that would cause students to be dismissed from the school.

For the application to be processed by the CCS Registrar's Office, it must be completed and submitted with the following items #1-10. (Please note that an application will not be processed unless all required forms are attached.) Please send all completed applications to the registrar at the school's address: 5955 Taylor Mill Road, Taylor Mill, KY 41015.

Upon receipt of the completed application and all necessary forms, parents will be contacted to schedule an admission interview and entrance testing.

NOTE: If no class roster opening is available, parents may request for the application fees to be returned or to have the student placed on the waiting list.

IMPORTANT: Items #1-10 must be completed and attached to the application before submitting to the Registrar.

  1. Application form completed and signed by parents or guardians.
  2. Non-refundable application fee.
  3. Pastor's Referral form for families applying for grades Pre-School - 12.
  4. Youth Leader's Referral form for students in grades 7-12.
  5. Student Questionnaire for students applying for grades 5-8 or 9-12.
  6. A copy of the applicant's birth certificate.
  7. A copy of the applicant's most recent report card.
  8. A signed Agreement Statement (4/08) - found on page 2 of the Parent/Student Handbook. (Parent/Student Handbook is to be completely read by parents and students grades 7-12.)
  9. Copies of the applicant's school records including standardized test results.
  10. Immunization and physical forms. Note: Immunizations must be documented on a Kentucky State Immunization Form provided by Kentucky doctors and/or the Kentucky Health Department. Physical forms are provided by CCS. (If these forms cannot be obtained from your doctor in a timely manner, a signed letter on the doctor's office stationery, stating the specific date of a scheduled appointment for immunization shots/or a physical, will temporarily suffice.)
